Include the device and browser you used, what you were doing, what you expected, and what happened instead. If you can reproduce the issue, a short sequence of steps is especially useful. A screenshot can help, but remove personal information first.

For a privacy or content concern requiring the publisher’s attention, include the relevant page address and a short description when a verified support channel is available.
